What are Accountability Structures?
Accountability Structures are the formal and informal governance mechanisms that determine who is responsible for verifying that AI-assisted decisions are grounded in shared operational reality. They define the human decision-makers, verification checkpoints, and escalation paths that prevent AI-generated outputs from circulating unchecked through the organisation's execution pipeline.
Why do traditional accountability structures fail under AI acceleration?
Traditional accountability was designed for human-generated work at human speeds. When AI generates outputs faster than governance can process them, the gap between decision velocity and verification capacity widens. Accountability diffuses: everyone assumes someone else validated the output. The result is a structurally unaccountable organisation — not through negligence, but through a speed mismatch that no individual can bridge alone.
How are Accountability Structures scored?
Scored 1–5 as dimension 3 (d3) of the Execution Integrity Score v1.0. A score of 1 indicates that no explicit accountability exists for AI output verification — decisions are made without named responsible parties. A score of 5 indicates that every AI-assisted decision domain has a designated human accountability owner, clear verification protocols, and documented escalation paths.
